Cass County makes IRS health coverage tax forms available on request

Cass County has announced that IRS Forms 1095-B and 1095-C for the 2025 tax year will be available to employees and former employees upon request. This process is in line with requirements set by the Affordable Care Act and the Paperwork Reduction Act, which allow organizations to provide these forms only when requested rather than sending them automatically.

The forms detail the health coverage offers made to employees, including those who have participated in Cass County’s Healthcare Savings Plan (MERS HCSP). Both current and former employees can obtain their copies of Form 1095-B or 1095-C free of charge.

Requests for these documents can be submitted in writing, by phone, or via email. Individuals seeking a copy should contact Cass County Payroll & Human Resources at 120 N.
