Dear Parents/Guardians of Bloomingdale Public School Students,
If you are enrolling a new student in Bloomingdale Public Schools, we are using an online registration format this year called Final Forms. You can begin your registration process by clicking the "Student Registration Tab" on the Bdalecards.org website. You can upload document or schedule an appointment with your child's school registrar through email or a phone call.

Required Documents for Enrollment
1) Child(rens) Birth Certificate or Passport
2) Parent/Guardian Driver's License or State Issued ID Card
3) Student's Immunization Records
4) Proof of Residency (Two of the following: Tax Bill, Mortgage Coupon, Insurance Statement, Declaration Pay Check Stub, Bank Statement, Signed Purchase Agreement with Property Owner or signed Lease Agreement with Property Owner.
